About Re-Birth Community Outreach Initiative

Our Mission

At RCO-INITIATIVE, our mission is to provide resources and support to homeless and at-risk individuals in our community. We believe that everyone deserves easy and dependable access to food, resources, and spiritual counsel, and we work tirelessly to make that a reality.

Bridging the Gap Case Management Program

Overview

Bridging the Gap is a holistic case management program under the Rebirth Community Outreach Initiative designed to help individuals and families overcome barriers to stability and self sufficiency. Our mission is to connect underserved populations with the resources, education, and support they need to thrive. It can be mentally, emotionally, and financially.

This program focuses on bridging the gap between need and access by offering hands-on guidance, advocacy, and practical support from intake through completion.

Our Approach

Each participant begins with a one on one assessment to identify their immediate and long term needs. From there, a case manager creates a personalized action plan that includes achievable goals, consistent follow-ups, and access to community partners that can provide additional support.

Our case managers serve as advocates. Helping individuals navigate complex systems such as housing, healthcare, education, and employment while ensuring that no one walks through their journey alone.

Our Commitment

At Rebirth Community Outreach Initiative, we believe that with the right support, every person can move from crisis to stability. Through Bridging the Gap, we don’t just provide temporary help, we walk with individuals until they have the confidence, skills, and connections to sustain long-term success.

Program Goals & Impact

1. Increase Access to Mental Health Resources

We help participants connect directly with licensed mental health professionals, therapy services, and support groups. Our case managers serve as advocates for those who may need help navigating insurance, scheduling appointments, or accessing affordable counseling. We also provide education on recognizing signs of stress, trauma, and burnout, encouraging overall mental wellness.

2. Provide Structured Case Management from Start to Completion

We guide clients through every step, from initial intake to sustainable independence.
Each participant receives:

  • A full intake and needs assessment
  • A personalized action plan with SMART goals
  • Weekly follow-ups for 6 weeks
  • Referrals to resources that meet their specific needs (housing, food, employment, or counseling)
  • Life skills, resume writing, and financial literacy training

Our goal is for each client to leave the program sustained, gainfully employed, financially stable, and equipped with the tools needed to maintain independence.

3. Build a Sustainable Referral Network

We are developing strong partnerships with local organizations, schools, and healthcare systems to create a well-rounded support network:

  • Schools: We assist parents and guardians in advocating for their children’s educational needs and connect families with tutoring, counseling, and literacy resources.
  • Hospitals: We partner with hospital social workers and programs that address mental health, substance abuse, and wellness care to ensure clients receive continued treatment and recovery support.
  • Local Offices & Community Partners: We work with agencies such as Workforce Solutions, Dress for Success, NAAM, food pantries, and local temp agencies to provide practical employment and resource connections.

4. Equip Individuals and Professionals Through Quarterly Summits

Each quarter, we host community centered summits that offer education and tools for both individuals and service providers. Topics include:

  • Financial literacy and budgeting
  • Mental health awareness and advocacy
  • “Know Your Rights” legal education
  • Employment readiness and workplace success

These events are open to the public and serve as opportunities for growth, networking, and empowerment.

Types of Assistance Provided

Bridging the Gap offers a range of direct and referral-based assistance, including:

  • Food and clothing assistance
  • Household necessities (furniture, toiletries, small appliances)
  • Housing and rental resource referrals
  • Job search and workforce connections
  • Financial literacy and life skills training
  • Mental health support and referrals
  • Community resource navigation and advocacy
